These are the projects funded in the health and social care sector:
Project: Developing the PA workforce to better support ill, disabled adults and children, and those at end of life. Development of a standardised training model, records and passports, as well as a an online database for Greater Manchester.
Project: Testing of novel training for health and social care staff supporting those in crisis and transition utilising Immersive Learning Experiences (performance, audio, visual art) to increase training.
Project: Housing association led training, coaching and in work support pilot project to support long term unemployed housing association tenants into health and social care jobs.
Project: Case study/ trial to integrate the "Greater Moments" care management app into a paper-based care home to streamline information reporting and increase resident-family interaction.
Project: Development of a prototype tech portal to streamline referrals from hospital discharge into post hospital discharge support from the commissioned voluntary sector.
Project: Development of care management software solution for the social care sector designed to improve operational efficiency by streamlining workflow processes with our novel rostering and care planning tool. While also promoting person-centred care, data-sharing with local authorities/inspectors and encourages partnership working.
Project: Development and testing of novel community-based engagement approach to system change and working with older people to improve health and social care services in North Trafford.
Project: Project to explore the benefits of increasing care related data availability for carers, care teams and recipients of care in a co-operative care organisation in order to empower the workforce and facilitate decentralised decision making.
Project: Testing the novel ‘Spreading our Net’ approach to addressing loneliness within local communities, focused on strengthening sustainable local networks between existing assets to connect people within neighbourhoods.
Project: Pilot implementation of digital solution to support multi-disciplinary teams providing care to residents with diabetes and other long term conditions in care homes, helping to assure equitable access to digital advances in care delivery.
